From the intersection of Colonial Dr. and Orange Ave., proceed east on
Colonial Dr. to Bumby Ave., Turn left... from the 408 (East West
Expressway) going west, exit at the 1st unmarked exit (Crystal Lake)
after the Holland East toll booth. Proceed west until you reach Bumby
Ave., then turn right... Proceed north on Bumby to Conine Dr., turn
right. Proceed east on Corrine Dr. to General Rees Rd., turn left.
Proceed north on General Rees Rd. until it dead-ends on Glenridge Way.
Turn right, and the school is on the right. Make a right onto Upper
Park Rd.

Enrollment
& Registration
All documents need to be complete
before you enroll your child.
1. Proof of residency
Owned residence:
A copy of the recorded deed or agreement for the deed or a certified
copy of declaration of homestead exemption and current utility bill and
driver’s license as proof that parents/guardian own and live at the
residence.
Rented or leased residence:
A copy of the lease or rental agreement, notarized letter from
landlord/ complex, and current utility bill and driver’s license as
proof that parents/guardian live at the residence.
Non primary owner or renter:
A notarized statement from primary resident indicating resident status
plus their lease or mortgage agreement , and current utility bill and
driver’s license as proof that parents/guardian live at the residence.

2. Proof of Guardianship
Divorced Parents:
A certified copy of the final judgment of divorce, court custody order,
a court guardianship order, or other such document establishing the
right of custody.
Separated Parents:
A notarized statement that the child actually lives in the home of the
parent designated in the child’s school records as the residential
parent
Foster Parents:
Department of Children and Families placement letter
Educational Guardianship:
Given only when the parents or custodial parent lives outside of Orange
County and adjacent counties.
3. Birth Certificate
4. Shot records transferred to a Florida Blue Health form
5. Copy of Physical –must be within one year from the state
of Florida only
6. Copy of last report card, exit grades, and test scores
(for proper placement)
7. Declaration of Domicile (if in Florida less than one year)
8. Must have withdrawal form from last school ( Florida ONLY
during school year)
9. ALL records must be translated into English ( out of
Country ONLY)
10. ESE Information if applicable
